Who is Eligible

Active AFOS member.*
Active AOA member. *
Junior nominee must have less than 10 years of service or less than 10 years from the date of graduation from optometry school.

 

 

*An active AFOS/AOA member is a member who is in “good standing”. Already applied and dues are up to date at time of submission.

Individuals, organizations, civic, and community services.

If the nominee is a member of any Federal Service, the individual must be an active member of AFOS and AOA.*

Who Nominates

The nomination is typically written by member’s senior officer, but may be written by anyone who knows the member’s qualifications.

Any active AFOS member may submit a nomination.

Submission Deadline

15 NOVEMBER : All submissions must be e-mailed to Dr. Carol Rymer at carol.rymer@us.army.mil The award recipients and nominees will be notified the first week of JANUARY to allow planning for the award ceremony at SECO in February.

Cover page: brief summary of title, job description, length of time in service, demographics (Name, Rank, Address, Duty Station, Phone, Fax, and E-mail)

Bullet page: limit of 2 pages of accomplishments, bullet style, via E-mail only. Exclude name for anonymity.

Judging Panel

5 Active AFOS optometrists who have been members of AFOS for at least 2 years. These members are selected from various branches of service.

AFOS executive council votes on the nomination. Maximum of 3 recipients per year.
